4-H Members and Leaders Forms

• 4-H Member Enrollment Form Online  - Required annually. Fee is $15/youth per year with a maximum of $30 per family. Letter requesting waiver of fee is available. Late fee of $10 per youth for previous 4-H members who enroll after December 1.
• 4-H Leader Enrollment Form  - Required annually. For adults who are currently enrolled as a 4-H Leader and have a current background check on file less than three years old.
• 4-H Projects List  - Can be used to select projects when enrolling for 4-H.
• 4-H Leader Enrollment Form background check needed--(for new leaders and those with background checks older than three years)--Required for adults who have not been enrolled as a 4-H Leader in the past year and/or those whose background check was more than three years ago.
• 4-H Permission Medical Release Form
• 4-H Code of Conduct Form
• 4-H Event Dress Code
